Attention Parents/Guardians: An Intramural Release AND Activities Permission form are required for all athletes, even if they are signing up for tryouts. Forms must be submitted to Coach King or the coach of the sport, prior to the first day of tryouts. An activities permission form must list all of the clubs/activities the child has permission to attend or a new form will need to be completed.
